Yale NewHaven Health is Connecticut’s largest healthcare system. Yale New Haven Hospital is a non-profit, 1,541-bed acute and tertiary medical center.
Yale New Haven Hospital, working with Yale Medicine, was the first hospital in the state to use technology that allows patients to have live “video visits” with their physicians or other healthcare providers. Yale Health ramped up 5,000+ video visits per day within EPIC EHR.
Yale’s Department of Radiology decreased their no show rate by 31% using Rx.Health’s Digital Navigation Pathways.
Rx.Health provided them an enterprise-wide unified platform to bridge the healthcare gap by providing digital solutions like online scheduling and screening, telehealth and mobile apps, direct messaging between patients and providers, and prescribing digital therapeutics.

This project helped inform the Health Provider of which patients had incorrect phone numbers logged in their EHR system
The Health Provider saw a 10.81% adoption of patient portal after 7 days post messages being delivered. This surpassed their annual target
